Please make sure you have all information and supplemental documents BEFORE starting this application.

For renewal applicants, you will need to submit:


1. Your current grade report (include at least one semester, two quarters, or one trimester).
2. A letter from an elder or minister which briefly describes your church involvement while in school this past year. It should be written on the church's letterhead.
